Introduction

Blossom24 Healthcare offers care and support, helping children and young people with a variety of physical, learning disabilities and mental health needs.

We offer planned and structured child-centred/child-friendly activities to the children at their family home or in a community setting. We provide bespoke packages of intense support to families with disabled children and/or significant additional needs. Our Local Authority Reports for children services and feedback from parents reflect exceptional success in this complex area of practice. We have experience and track record of working closely with other services such as health, social services, education, police and community organisations. 

Our aim is to ensure that children and their families experience positive outcomes from the care we provide. We consider Children’s physical growth, as well as their ability to learn the social, emotional, behaviour, thinking and communication skills they need in life. Our Support team has practical experience in dealing with challenging behaviour, autistic spectrum disorders, complex health needs, physical disabilities and sensory impairment. We recruit workers from a range of backgrounds and discipline and endeavour to match workers to specific cases.  This assists with the development of a positive rapport between the child and the worker and to ensure consistency for the family. Our service comprises of a small Specialist Management Team of Registered Nurses, Child and Adolescent Specialist Nurse and support workers who have relevant knowledge, experience and skills of working with children/young people and families

Children and Family Support Services

Blossom24 Healthcare provides  high quality, bespoke family support packages to children/young adults and families in need, whether for a few hours a week or regular more intensive support. Risk assessments are undertaken at the initial assessment meetings and reviewed on a regular basis. We design a Child Outcome Focused Support and Person Centred Plan to ensure we meet the needs of the children and families. Blossom24 Healthcare is able to meet the needs of children and reduce the need for children to be taken into care and reduce the burden and long-term financial commitment of Local Authorities. Care and Support takes place in variety of ways in the community and children’s home.

Home Care

Our Home Care Service Supports children and young people in their homes, providing day to day personal care, encouraging independence and daily routines including medication administration. Extra Support is provided at home to support families in a variety of situations. 

Perhaps parents will be attending hospital appointments and your child or young person needs looking after while you are away or maybe your child or young person is recovering from an illness, has disabilities or behaviours which challenge where you will need extra support and help. Whatever the reason, our trained and experienced support workers are there to protect and support children and young people through difficult times.

Community Enablement

We offer exciting community enablement across Northamptonshire. We support children and young persons to access a wide range of activities in a safe and stimulating community environment.Our activities are designed to allow children to build confidence and make new friends including gaining social skills on a 1:1 or 2:1 basis. This includes:

  • Community Access-Play Areas,Trampoline, Swimming, Outdoor/Indoor Gym, Climbing, Ice Skating
  • Outdoor Activities/Country Parks/Therapeutic Walks
  • Promoting independence/Travel Training
  • Visiting Theme Parks/Sea Side/Safari Park

Supervised Contact & Reunification

We support families to maintain links with both parents and other family members when the family breaks down. Our professional family workers provide support  through supervised contact meetings. Family support workers are are CRB checked, trained and attain professional standards with good customer care and punctuality. Family Workers are supervised by a Qualified and experienced professional through observations, appraisal, mandatory and competency training.

Parental Assessments

Blossom24 provides parental assessments based on Parent Assessment Training Manual developed by Dr Sue McGaw and this includes working with a number of families where one or both parents experience Learning Disability or Autism. We have experience of working in this area and offer a quality outcome service to these individuals.

Our Assessment covers: Child Care and Development, Behaviour Management, Independent Living Skills, Safety and Hygiene, Parents’ Health, Relationships and Support including the impact of the environment and community on parenting. Each parenting skill area within the domain is assessed for parental knowledge and quality of parental skills. After the assessment, the assessor will have a clear visual family profile that targets parenting support needs as well as child protection concerns. PAMMS techniques are also included in our approach for parents with Learning difficulty/disability and this includes: Breaking tasks into accessible steps, demonstrating tasks, use of pictorial aids such as pictures, diagrams, symbols, role play, short teaching sessions, verbal feedback, praise, confidence building and visualisation.
